Are you passionate about improving access to mental health care and making a positive impact on people's lives? We're looking for a dedicated and empathetic Behavioral Health Coordinator to join our team and play a vital role in our Collaborative Care Model (CoCM).

Understanding the Collaborative Care Model (CoCM)

CoCM is a dynamic, team-based approach that integrates mental health services into primary care settings. You'll be part of a collaborative team, including primary care providers, psychiatric providers, and other behavioral health specialists, working together to provide holistic and coordinated care. Our goal is to enhance patient outcomes and improve access to vital mental health resources within the community.

Your Role as a Behavioral Health Coordinator:

As a Behavioral Health Coordinator, you'll be at the forefront of providing crucial mental health support. Your responsibilities will include:

Comprehensive Assessments: Conduct initial assessments to understand patients' mental health status and identify their specific treatment needs, including screening for common conditions like depression and anxiety.
Collaborative Treatment Planning: Work closely with primary care providers, psychiatrists, psychologists, and other team members to develop and implement personalized, evidence-based treatment plans.
Direct Patient Support: Provide brief interventions, counseling, and psychoeducation to empower patients and support their mental well-being.
Progress Monitoring and Follow-Up: Monitor patients' progress and treatment adherence through regular follow-up appointments, assessments, and outcome measurements.
Seamless Care Coordination: Coordinate care transitions and referrals to specialized mental health services or community resources as needed, ensuring patients receive comprehensive support.
Patient and Family Education: Educate patients and their families about mental health conditions, treatment options, self-care strategies, and available resources.
Accurate Documentation: Maintain detailed and accurate patient records, treatment plans, and clinical outcomes in our electronic health record (EHR) system.
Team Collaboration and Quality Improvement: Participate in case load reviews, team meetings, and quality improvement initiatives to enhance the effectiveness of our integrated behavioral health services.
Community Collaboration: Collaborate with external mental health providers, social services agencies, and community organizations to address patients' psychosocial needs and promote holistic care.
Ethical Practice: Adhere to the highest ethical and legal standards, ensuring patient confidentiality and maintaining professional boundaries.
What We're Looking For:

Bachelor's Degree in Sociology or Psychology.
Proven experience as a Behavioral Health Specialist or Clinical Social Worker in a clinic setting.
Ability to thrive in a multidisciplinary team environment.
Strong understanding of health and patient care regulations.
Excellent communication and interpersonal skills.
