Care Coordinator – Empower Families Navigating Elder Care
About the Role
Are you a compassionate professional with a background in social work? Do you love helping families make informed decisions and supporting seniors with dignity — without the demands of clinical or case management work?
As a Care Coordinator, you’ll serve as a trusted advisor to families navigating the complexities of elder care. You’ll conduct assessments, clarify goals, educate on care options, and coordinate with care managers and professionals — all while ensuring that legal and care plans align. Training will be provided to help you understand the legal planning side of our work, so you can confidently connect health and legal planning in a way that empowers families. This is a collaborative, non-clinical role where your empathy, organization, and communication skills will shine.
What You’ll Do
- Guide families through care planning with empathy and clarity
- Conduct assessments to identify needs, goals, and concerns
- Educate families on care options, resources, and planning strategies
- Coordinate with care managers and facilities — not manage care directly
- Use tools like Microsoft Teams and Curo365 to document and collaborate
- Attend care planning meetings and advocate for client needs
- Represent the firm at community events and build strong professional relationships
What You Bring - Qualifications
- Bachelor’s degree in social work, healthcare, or related field
- 2–4 years of experience with seniors (hospice, senior living, care navigation, etc.)
- Strong listening, assessment, and communication skills
- Familiarity with elder care systems, dementia care, and caregiver support
- High tech fluency — if you struggle with digital tools, this role is not a fit
- Empathy, professionalism, and a team-first mindset
