Job Title: Case Manager (Three Rivers)
Hours: 40 hours/week - Monday through Friday 8:00am-4:00pm
Educational Requirements: High School Diploma or equivalent is required, or a combination of the following education and experience. Highschool diploma with one year experience working with individuals with developmental disabilities OR a high school diploma with a minimum of two years working experience in human services or a related field OR a Bachelor’s degree in a related field.
Other Requirements: Ability to lift 25 lbs. comfortably, a valid driver’s license and a personal reliable vehicle for transport of clients.
Job Summary: Facilitate and implement a comprehensive planning process designed to enhance all aspects of everyone’s life.
Salary: $24.96/hour
