As a Residential Case Manager, you will provide case management services to clients residing in the residential program.

Some of the Functions Include:

  • Participate in Treatment and Service planning and review meetings.
  • Review available records regarding the family’s history, including but not limited to the child(ren)’s educational, medical, dental and/or mental health history, etc. 
  • Ensure clients are enrolled in an accredited educational and/or vocational program.
  • Identify and explore potential resources that will facilitate family reunification, or alternative permanency planning. 
  • Develop and maintain chronological notes in the client’s file and/or Florida Safe Families Network (FSFN). 
  • Coordinate/conduct supervised visitations between parents and child(ren), as well as between siblings. 
  • Coordinate and monitor healthcare and dental examinations and services for clients. 
  • Actively participate in permanency planning to include attendance in case plan and permanency staffings held by the Full Case Management Agency.  Assist with communicating the needs and wishes of the client, ie. via use of the youth permanency input form.
  • Explain the court/dependency process to the child and ensure active involvement and participation.  Facilitate client’s attendance to court and required staffings.
  • Staff cases with Treatment team, Lead Agency, FCMA staff, and other professionals as required.
  • Coordinate, monitor and track client’s progress with learning life skills, including but not limited to independent living assessments, trainings, monthly logs, and required staffings. 
  • Testify at court or Citizen’s Review Panel (CRP) regarding reunification, termination of parental rights, or other permanency plan for children. 
  • Provide support to clients to assure stability and timely achievement of permanency. 
  • Assist family and/or caregivers in accessing subsidy, medical coverage, and other needed services for children.
  • Maintain regular communication with client’s FCMA Case Manager, parent/caregivers, Guardian Ad Litem, and all necessary parties.
  • Participate in Discharge and Aftercare planning
  • Monitor the aftercare plan and provide aftercare services as required by the Florida Administrative Code and applicable CFCE policies.
  • Conduct and document required contacts/home visits as required based on residential setting.
  • Coordinate the delivery of services to children and families by providing appropriate referrals and ensuring linkage of those services to clients. 
  • Provide written progress report to FCMA Case Manager every 30 days for client receiving aftercare services upon discharge from the therapeutic group home.
  • Arrange and/or provide transportation for clients, when needed.
  • Provide on-call support for clients as required or needed. 
  • Participate in the Continuous Quality Improvement process.

Minimum Education/Experience

  • Bachelor’s Degree in a human services related field from an accredited university,
  • OR a Bachelor’s Degree in a non-human services related field and Twenty-four months of relevant experience working with children and families
  • At least one year working in a Residential program setting
  • Must possess a State of Florida Child Welfare Certification
